The school will not give your child medicine unless this form has been completed. Where possible, medication should be given at home by a parent. Where this is not possible, please adjust the timings of medication being administered so that it only need be given at school once.

Declaration:
The above information, is to the best of my knowledge, accurate at the time of writing and I give consent to school staff administering medicine in accordance with the school policy.

I will inform the school immediately, if there is any change in dosage or frequency of the medication or if the medicine is stopped.

I understand that I must deliver the medicine personally to the school office.

I will collect the medicine from the office when the course finishes or my child does not require it any more.
